Cost of a Home Sleep Test

Your dentist will discuss the cost of your oral appliance therapy and payment terms available. If you are obtaining your home sleep test through SleepArchitects and REST Diagnostics, and you have medical insurance, SleepArchitects will work with your dentist to find out what coverage may be available from your medical insurance provider.

Medical Insurance

All services are subject to medical necessity, contract limitations and exclusions (e.g. payment of premium). This is not a guarantee of benefits and all claims are subject to the terms, exclusions, and conditions at the time of service. Benefits are given to patients based on information received by your insurance plan. If the benefits are different than the terms in the member’s contract, the terms will take control. This is an agreement between REST Diagnostics, LLC and the Patient/Guarantor/Legal Guardian. In this agreement the words you, your, and yours mean the Guarantor/Patient/Legal Guardian, responsibly party. The word ‘account’ means the account that has been established in your name to which charges are made and payments credited. By executing this agreement, you are agreeing to pay for all services that are received. You authorize the use of this signature on all insurance submissions whether manual or electronic.

When we submit your claim for medical reimbursement, you may receive one or multiple documents from your insurance carrier known as Explanation of Benefits (EOBs). An EOB is not an invoice. It is simply an explanation of how our services are being applied against your benefits. Please contact us at (949) 347-5070 if you have any questions.

Financial Responsibility for Cash Services

Payment is expected on the day you schedule delivery of the Home Sleep Test Device. You may pay by check, debit card or credit card.

Sleep Study Cash Price: If you do not have medical insurance or your medical insurance does not cover the test, the out-of-pocket cash price for the Home Sleep Test will be provided during your scheduling call with SleepArchitects.

Return Checks: There is a $30.00 fee on any checks returned by the bank.

Additional Fees & Return Policy

Return Date

You will be given a Shipping Return Deadline when you speak to a SleepArchitects representative to schedule your test. A confirmation of this Shipping Return Deadline will be sent to you by e-mail or fax. The return box must be postmarked on or before the Shipping Return Deadline to avoid any late fees.

Late Fees

A late fee will be issued for every day the device is not returned ($150.00 per day), unless arrangements are made with us prior to due date. If the device is not postmarked within 3 business days after the Shipping Return Deadline then the patient agrees to pay for the purchase price of the device.

Shipping Cost

A UPS return label is being provided to you. Please repack the device in the box it came in and drop off the box with the UPS label at a local authorized UPS facility. Note that UPS is not the US Postal Service (USPS). If we incur additional shipping charges, we will send you an invoice for those additional services (i.e., Saturday pickup, delivery pickup, etc.)

Lost or Stolen

If the device is lost/stolen while the patient’s responsibility, the patient will be responsible for the purchase price of the device which can be up to $4,000.00.

Acknowledgement

Upon receipt of this agreement, you acknowledge having read and complied with all the terms and conditions contained herein. For collection purposes, we have the option to report your account status to any attorney, collection agency, or credit bureau.

Other Sleep Test Providers

If you are obtaining a sleep study through an organization other than SleepArchitects and REST Diagnostics, you will need to work directly with that sleep study organization regarding use instructions and cost.

Insurance FAQ

We work with most private PPO plans, including all of the major insurance carriers, such as AETNA, Blue Cross/Blue Shield, United Health Care, Cigna, Humana, and any other PPO carrier.

The coverage depends on the benefits plan you purchased.  Normally, you pay 100% of your individual deductible before your medical plan begins to cover additional medical costs.  Most dental providers fall out-of-network relating to dental sleep therapy.  To understand your out-of-network coverage, go to your plan’s website for more information, or call the customer phone number on your medical insurance card.

Yes, you can make arrangements to use your FSA/HAS Spending Card with your dental sleep provider.

We are able to service sleep testing to patients with Medicare coverage.  For other types of insurance, please provide front and back copies to your medical insurance card so the appropriate verification of benefits can be completed.  While we are not able to work with Medicaid, TRICARE, or other HMO plans, we do as much as we can to help you through discounts and our convenient cash-pay service.
